Organophosphate Poisoning
A 35-year-old male is brought to the emergency department (ED) after being found unconscious in a farm field, where he was handling pesticides. Upon arrival, he exhibits respiratory distress, excessive salivation, miosis (pupil constriction), and general muscle twitching. His vital signs show bradycardia (heart rate of 50 bpm) and low blood pressure (90/60 mmHg).
Plasma Cholinesterase Levels: This is the investigation of choice
Atropine and Pralidoxime as Rx
Toxoplasmosis
A 30-year-old immunocompromised female (HIV positive, CD4 count of 150 cells/mm³) presents to the emergency department with complaints of sudden onset headache, visual disturbances, and fever. A CT scan of the head shows multiple ring-enhancing lesions in the brain, raising suspicion for cerebral toxoplasmosis.
Serologic Testing for Toxoplasma IgG Antibodies
Pyrimethamine and Sulfadiazine
